Sewer Gas

I have a sewer gas odor in the main floor half bath (of a two story house). We have tried all the usual suspects: We frequently use the sink, so the trap is not evaporated dry; we replaced the wax seal on the toilet; when that didn't work, we replaced the flange and another new wax seal. (All of this was done by professional plumbers...two of them, just in case we got an incompetent one the first time.) There is a full basement under the bath. We removed the insulation under the drain; there is no evidence of leaking, and no sewer smell there. In the attic above the bath (actually two floors above, since there is a finished second floor room in between) there is also sometimes a sewer smell. The only time the smell disappeared was when the toilet was removed, and a test plug was in the soil pipe. What else could be the problem?
